FOR WHOM:
This award will be given to a volunteer who is a member of the medical family and has made a significant contribution to a health related issue in California. It will be a one-time award for a participant and must be submitted by a CMA Alliance member.
HOW TO APPLY
1. Submit the name of a prospective recipient.
2. Fill out application page.
3. Write a one-page essay explaining why this applicant should receive this award.
WHEN:
1. Deadline July 1, 2008.
2. Selection will be made by the CMA Alliance Executive Board on July 15, 2008
WHAT IS EXPECTED OF THE WINNER:
1. Attend the event on Sunday, October 5, 2008 at 6:30pm in Sacramento, CA
2. A short 2 minute thank you speech
RECIPIENT WILL BE HONORED AT THE EVENT WITH:
1. Recognition plaque
2. Complimentary CMA Foundation dinner
